How to Complete the General Form for Insects

In many cases the identification of an insect or a spider alone does not answer the question of why the arthropod was present or from where the problem originated. Knowing this information will help the diagnostician to prescribe management recommendations.

To help provide a more complete diagnosis, please include the following information:

  • How many of the pest in question have you seen?
  • Where do you see them (in what room of a building or on what plant, etc.)?
  • How often or at what time of the year do you see them?
  • Is the area wet?
  • Is the item new?

Any information about the insect or spider (including images showing damage or location) is greatly appreciated.
